One of those resources is the GUIDE Model, a program created by the Centers for Medicare & Medicaid Services (CMS).

What Is The GUIDE Program?

GUIDE stands for Guiding an Improved Dementia Experience.

The GUIDE Model is a nationwide CMS program designed to provide comprehensive, coordinated support for people living with dementia and their family caregivers. The goal is to help people living with dementia remain safe in their homes longer while also supporting the health and well-being of the people who care for them.

The GUIDE program also seeks to coordinate care through care management with evidence-based tools and support, with the goal of improving quality of life and reducing more costly care outcomes.

Through GUIDE, eligible participants can receive access to a range of services, including:

  • A dementia care navigator who helps coordinate care
  • Regular care coordination and personalized care planning
  • 24/7 support line for urgent questions and concerns
  • Caregiver education, training and support
  • Connections to community resources
  • Respite services for eligible participants

Why Respite Matters

Caregiving can be a 24-hour responsibility. Even the most dedicated family caregiver needs time to rest, work, run errands, attend appointments, or simply recharge.

Through the GUIDE Model, eligible participants may receive up to $2,500 annually in respite services. CMS allows those respite services to be provided in several settings, including in-home care, adult day centers and facility-based respite.

Respite is more than simply giving a caregiver a few hours away. It can allow families to build a sustainable caregiving routine, one that allows the caregiver to continue providing support without becoming overwhelmed.

The Connection Between GUIDE And Adult Day Services

Adult day services can be an especially valuable form of respite.

Rather than spending the day alone at home, a person living with dementia can spend time in a supportive environment designed around connection, engagement and meaningful activity. At the same time, a family caregiver can have time to work, take care of personal responsibilities, attend appointments or simply take a much-needed break.

At Clermont Park, our Adult Day Program provides older adults with opportunities to grow, connect and participate in a supportive community while giving family caregivers a break from their caregiving responsibilities.

Clermont Park is proud to partner with CU Medicine, a participant in the GUIDE Model, to provide Adult Day services as a GUIDE respite option. For eligible participants, GUIDE provides up to $2,500 annually that can be used toward respite services, including participation in Clermont Park’s Adult Day Program.

Who May Be Eligible?

Eligibility is determined through the GUIDE program and Medicare. In general, CU Medicine’s GUIDE Program serves people living with dementia who have traditional Medicare, not Medicare Advantage, and who live in a home setting or independent living. The person living with dementia must also live in the Denver metropolitan area or surrounding areas served by the program.

Clermont Park’s materials identify the following general Medicare requirements:

  • Enrollment in Medicare Parts A and B, with Medicare as the primary insurance
  • Not being enrolled in Medicare Advantage
  • Not having elected Medicare hospice benefit
  • Not residing in a long-term assisted living or nursing home

Respite eligibility is determined by Medicare as part of the GUIDE enrollment process, so not everyone enrolled in GUIDE will necessarily qualify for respite services.
